Data Tracker for Engagement & Outcomes

Build a lightweight, privacy-aware system in Google Sheets and Forms to track engagement, outcomes, and surveys for our community-based mental health care program. The goal is to measure success, improve access, and share evidence with funders.

Deliverables include:
- A Google Sheet for monthly EHR exports that auto-calculates KPIs (e.g., Days to First Session, ≥3 Sessions, Attendance/No-Show, improvement indicators).
- A one-page KPI dashboard with simple equity views.
- A survey sheet with tabs for contacts, anonymous responses, and an admin-only code map linking ClientID for equity analysis.
- If time: a plain-English SOP for running the system with minimal staff.

FACTS Initiative brings trauma-informed, culturally responsive therapy into trusted community spaces so families can start sooner and stay engaged. This project helps us track and improve outcomes equitably and privately.

Inputs (sample exports, current tracker, privacy language) and Drive access are ready. Weekly 30-minute check-ins and quick feedback ensure progress. Success criteria: working dashboard, anonymous survey pipeline, and usable SOP.
